Swelling at the angles is often due to fluid retention in the body. If both ankles are swollen that indicates excess of body fluid either due to immobility or due to other conditions such as heart disease, kidney or liver conditions. It is best to have her checked up by a physicians. In the meantime cutting down on salt intake may help to reduce the fluid retention.
