Sharp pains lower left abdomen, mucus in pee, pelvic pain, micro hematuria, smelly cloudy urine, urethral pain and hard time peeing. clear ultrasounds earlier this year, elevated platelets, several pelvic exams clear, no uti it std, what could it be?

You did not mention urine C&S and Kidney ultrasound. For all this you need to see a Urologist for proper exam and treatment, for it could be nonbacterial cystitis, stone or something else In the meantime pl drink plenty of water and keep the vulvo- vaginal area clean and take some AZO. Good luck
