If an ent has inspected the tonsils and found no irregularity, how long afterwards would it realistically take before a tonsillar cancer grew and could be visualized?

From days to years. It has to do with the aggressiveness (growth rate) of the cancer and the fact that changes may start with a different process, such as infectious or mechanical/ chemical irritation that turns in time in a malignant process.
