Nerve impingement or bilateral neuritis are possible causes. Has a neurologist evaluated him? An EMG seems indicated if not already performed. An MRI of neck vertebrae may shed light. If already seen by a neurologist, I recommend consulting another at a department of neurology at a university hospital near you. I am confident that a definitive diagnosis will be obtained with a proper testing.
