It really depends on how much Ventolin you are taking. If you are taking 2 puffs up to four times daily, you will likely not have an issue with irregular heartbeats with azithromycin. If you are taking more of the inhaler than that or you experience a racing heart, tremors or feel skipped beats when using the Ventolin inhaler, then speak with your doctor before taking the azithromycin. The products may affect the heart rhythm by different mechanisms. Ventolin can accelerate the heart rate while azithromycin can prolong the QTc interval on an EKG setting the stage for dangerous ventricular arrhythmias especially if taken with other meds that can have the same effect. If your QTc interval is less than 460 & you& family has no history of dangerous arrhythmias you have little risk
