First reading was 145/100, then seconds later blood pressure is 118/91 pluse which one is accurate?

Either may be accurate or neither may be. Readings are more likely accurate when using a standardized method with verified equipment. In the ICU we use a catheter inserted in the artery that yields continuously accurate readings. Different readings close together suggest a variation in technique/declining battery power/or other technical glitch.
