It sounds like you are confused and need to discuss the issue with his doc. We worry about narrowed arteries in the neck, not veins. If they get too narrow in spite of treatment, they consider a bypass surgery. You should really clarify the issues with his doc.

Hard to interpret your post, but would assume that the MRI report might have implied presence of "microvascular angiopathy". If he is experiencing symptoms consistent with transient ischemic attacks, there are several medications which could be employed for prevention of stroke. An experienced neurologist could capably put this into context.
