Fluorosis occurs in childhood, up to 48 months of age. This problem is not caused by using too much toothpaste in adult life. By UPTODATE: according to data from the National Health and Nutrition Examination Survey, the prevalence of fluorosis among persons aged 6 to 39 years was 23 percent during 1999 to 2002.

Fluorosis is a process known to occur as the tooth is formed, not from fluoride toothpaste after it emerges. I would expect the 4 teeth involved were formed during a time when you were drinking water with a high fluoride content, as occurs in various regions.
