You’ve had more than one urinalysis over the past year, but despite the flank pain and hematuria, you haven’t had any further evaluation? My first guess is kidney stones, but there are other potential causes of your symptoms that really need to be excluded. Please schedule a visit with your doctor just for this problem so the doctor can focus on your specific symptoms.

Kidney pathology wouldn't cause those types of back symptoms, but spine pathology would. To evaluate microhematuria, first collect the clean catch urine sample carefully so that vaginal contents won't contaminate it. A culture,colony count and microscopic exam of the sediment & cytology exam to look for malignant cells next &if microhematuria is present ultrasound of kidneys& bladder then advised.

The cause of blood has to be investigated. It may or may not be related to the pain. A good physical exam could also determine whether it is a back problem .
