Congestive heart failure is a clinical diagnosis and can be suspected by a careful history. Body (heart) is talking to you. Listen and take good notes. Starts with ankle edema at end of day. Then nocturia. Then waking up at night short of breath or coughing up blood. Some like to have high BNP levels. Discuss with team in Guam.
