What does it mean if my vitamin b12 blood test level is 1,261 pg/ml? it was fasting blood work, i don't take vitamins. neurologist didnt seem concerned but it showed as a abnormal result?

All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Talk to the doctor who ordered the tests. Having said that, elevated B12 may be an indication of chronic inflammation, though B12 by itself is harmless. Wish you good health!
