It can most certainly be a cavity. The enamel is very hard, so the carious lesion can start very small, but once it passes through the enamel the dentin is much softer so it can progress rather quickly. Please see a dentist to have that tooth evaluated and restored.

A small hole not dark but tooth sensitive to biting means something is wrong. The hole may be small to look at but have a large cavity when opened up. Sensitivity is also a sign that you have a cavity, now see a dentist for treatment before you lose your tooth.

Sometimes under a pinhole opening in the enamel is a large cavity (infection). Sensitivity indicates infection is approaching the nerve of the tooth. You need a Dentist, NOW, before sensitivity turns into roaring abscess.
