Probably. It's called a Buccal Pit. Damage to the dentin under the enamel can be quite extensive. Trying to use a toothpick is like trying to get your elbow into your ear canal, the pit in the enamel is usually quite small. Please see a Dentist for competent professional evaluation/treatment.
