The pain described may also be caused by a fracture. Go to an emergency room an exam , x-ray of arm and proper cleansing.

You need to see a doctor or a surgeon ASAP. You wont be able to do the cleaning yourself. Also make sure your tetanus shot is up-to-date. I hope you do well
