Hello i was wondering if a physician on this service would be able to fill out a medical evaluation form during a telehealth visit with me. i need to have this form completed to study overseas?

A health form for overseas study probably requires an in-person examination. One would need to evaluate the heart and lungs, and vital signswhich is rarely able to be done online (you need the technology, which is found only on a few telemedicine sites)

You need an official document that might have clear requirements such as vital signs or review of documents such as vaccination history that will make it unsuitable for a telemedicine consult. Would suggest your primary care doc who has access to your medical history.
