Ribs attached to the sternum with cartilage may become dislocated with sometimes un-recognized trauma and do not reattach firmly. Movement which accentuates this "non-anchored" rib will produce point tenderness and the pain described (as long as potential cardiac/GI issues have been "ruled out").
