Baby head measurement at birth was 35.5cm & 39cm at 1 month old. can the drastic change in percentile mean hydrocephalus?

The head circumference reading should be repeated by your pediatrician's office and pediatrician. Very often there is a reader to reader variability. Per the growth chart there has been a rapid increase, more than expected, and a follow up head ultrasound or imaging study may be indicated. Is the baby otherwise well? are the other growth parameters also changing rapidly? Watch for poor feeding.

This is within the upper normal range for infants at that age. Measurements will also vary some by who does it and can vary at the same encounter. The pediatrician can also readily discount the idea "hydrocephalus" or other expansion under pressure by simple measures on physical exam.
