Intense intermittent fatigue (esp. after meals), muscle twitching all over, more frequent pvcs for 2 months. plant-based diet for 3 years. ionized calcium was just measured as 1.22mmol/l (1.19mmol/l adjusted to ph 7.4) - does that seem a bit low?

All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Talk to the doctor who ordered the tests. Having said that, normal value is 1.20 to 1.40 millimol/L Wish you good health!
