For a person with normal kidney function, primary hyperparathyroidism causes high PTH level and high serum calcium (HYPERcalcemia). You do not state you have chronic kidney disease but the most common cause of high PTH and slightly low calcium (mild HYPOcalcemia) is moderate to advanced chronic kidney disease. Vitamin D3 level can be normal but active D3 level could be low.
