I've been having tonsil problems for a year and half, they have multiple lumps on them and one is bigger than the other i also get on and off sore throats, i'm also a smoker of 10 years should i get a cancer screening?

It's best to see an ear/nose and throat specialist (otolaryngologist) for an examination of one's throat, head and neck areas to determine what is visible and what tests might be appropriate to clarify the underlying cause of these symptoms and signs.
