Have always had pvcs, pacs and runs of nsvt mostly at rest. lately it's occurring mostly with activity and sometines at rest. normal echo 1 year ago. does this mean anything sinister?

Normal echocardiogram does not rule out heart rhythm problem. If you feels more ectopic heart beats recently, you may need to have a holter monitoring or a loop monitor. It's time you visit or revisit a cardiologist.

You need to have an event monitor to evaluate the burden of arrhythmia ( percentage) and then decide if treatment is needed
