Urologist, if established with one, best would be with your primary care doc, who can examine, take a detailed history in the context of your known conditions and treatment and further coordinate as needed, order tests or refer to a specialist.

Would be fine to start with your primary care physician who could do appropriate blood studies, and imaging if necessary. Urology would be an excellent consultant to more fully evaluate status of prostate, but most likely this is benign prostatic hypertrophy causing the frequent urination. Medication may help, but surgery is sometimes necessary.
