I'm on my last day of antibiotics for tonsil infection and am noticing symptoms returning. what should i do? do i need to start antibiotics again?

If a sensitivity study was not performed on the initial tonsil (throat) culture, the antibiotic ordered may not be effective. See your physician and have a repeat culture/sensitivity done. Also get a COVID-19 test, as symptoms of COVID can present with throat irritation, pain, and antibiotics will not treat this virus.

Your prescriber has to be informed as among others, diagnosis has to be challenged. The majority of antibiotics have also an anti inflammatory effect and may help transiently. Cultures will have to be obtained in case infection remains likely.
