All of the above! Diabetes, obesity, thyroid issues and your advancing age can cause a reduction or cessation of ovulation and thus fertility. The first step is getting all of these medical issued seen by an Internal Medicine specialist and under control before seeing a Fertility GYN MD.

At "41", with weight at "290" with "thyroid issues" and "diabetes", these factors create significant risk in pregnancy. Get a referral to a maternal/fetal medicine specialist for counseling. Fertility is compromised at this stage of reproductive life, but your state of health is also a major concern.
