Thyroid ultrasound reveals slight increase in size of lobes from 3 5x1.2x.9 mm to 4.1x2.1x 1.2mm and isthmus is .1. no nodule. tsh slightly high inspite of synthroid.have hashimoto's is the increase abnormal? could it be cancer?

Your thyroid is currently hypoactive and is trying to make more hormone which causes increase in size. Your synthroid dose needs to be increased in order to bring your thyroid into proper range.
