Hemorrhoids are usually painless except unless they are thrombosed or prolapsed. Usually patients who present with the complaint of painful hemorrhoids have some other diagnosis (anal fissure most commonly) causing the pain in the region which they attribute to the hemorrhoids. While it’s ok for you to use over the counter hemorrhoidal creams in short term, checking in with your doctor is best

I agree with Dr.G. In my experience a thrombosed hemorrhoid is the usual cause of such pain and,if so, the clot has to be removed with a small incision. For this reason I recommend consulting a proctologist or colorectal surgeon straight off. In the meantime apply warm compresses. A perianal abscess is also best handled by these specialists.
