Someone needs to take another look at that ear. Don't take any more ibuprofen (max daily dose is 2400 mg) or Tylenol (2000 mg/day for several days can be dangerous), exceeding an adequate dose doesn't help. Sometimes a middle ear infection (otitis media) is diagnosed as a swimmer's ear (otitis externa) and vice versa. The treatments are different. Go to the ER, that why they're there 24hrs/day.

There are several approaches which may provide answers, such as a second opinion from an otolaryngologist, a visit to an urgent care center, evaluation in an ER. If no infection found, and no structural changes demonstrated on imaging, you might wonder about a rare condition, "nervus intermedius neuralgia", which might be treated by a pain specialist.
