"Iron related anemia" in a male may not be related to iron stores or ferritin levels, but inadequate production of red cells by bone marrow or organ failure. Attrition of red cell life occurs constantly, and cell production as well as loss of blood via hemorrhage may be responsible for the anemia.

Iron deficiency anemia in an adult warrants that internal bleeding be excluded. There may not be obvious blood in stool. It may be as simple as hemorrhoids or something more serious. It would be prudent to consult a GI specialist. Wish you good health!
