Fibromyalgia is one possibility but muscle spasms aren't common. I recommend consulting a specialist in internal medicine who will take a thorough history & system review, perform a complete physical examination, & order appropriate tests. You may then be referred to other specialists such as a neurologist or rheumatologist depending upon the assessment.

Cervical myelopathy could explain the symptoms and would need prompt attention especially if there were additional bowel and or bladder complaints. Another consideration, which is less serious but still very aggravating, is fibromyalgia. Would consider seeing a physical medicine and rehabilitation (PM&R) specialist who could fully assess and treat either problem.

Agree with Dr Tamler, but there are a few more causes of total body pain: Diffuse small fiber neuropathy, complex regional pain syndrome (which has centralized), and also, diffuse arthritis and fibromyalgia.
