I was helping a patient put his leg on the bed , he scratched my wrist with his sharp toe nail and my wrist started bleeding , i cleaned it with an alcohol pad . is it possible to get hep c, hep b or even hiv from the patient?

It is unlikey you will get any of those diseases , unless the patient is positive for those diseases,If the wound is deep you may need antibiotics , otherwise keep the wound clean and if necessary bandage it

Even if patient is a carrier of one of the diseases you mention, it's unlikely the virus is present on his toenails and infests your body through the cut.
