Both of these are common ECG reports where there is uncertainty as to the cause ECG "abnormalities" since similar changes may also occur due to technical factors such as placement of he electrodes at the wrong site, variability in the heart position due to thoracic abormalities, ,electrolyte change setc. In my opinion it is always better to mention the "abnormalities" rather than make a diagnosis
