Varies from doctor to doctor and hospital to hospital. Your policy will tell where it is applicable. If you have time you can clear it prior to entering an office or hospital. If an emergency you may have no choice. Always remember, your insurance company loves you until you sign and pay. Then they are your worst enemy.

It has to do mostly with the office or facility billing you connecting with your insurance. You will most likely have a copay in the range of 20-30 $ regardless for a simple office visit.
