Not often: This has generally been legislated out of most plans although it was common in the past. Many plans, however, have time limitations so no coverage for many conditions including pregnancy for a period of 4-8 months after signing up to prevent people with no insurance or competing insurance from jumping into a new plan while already having a condition.
