The normal heart beat is regular and same volume with each beat. If you get a sensation of a "missed" beat, it is likely that you have what is called Ectopics or PVC's. When suspected, ectopics should be confirmed with an ECG/EKG (electrocardiogram). You should see your family doctor and get a check up of your heart. Most Ectopics are benign/harmless and go away if you cut down on coffee/Alcohol.

If you're referring to an irregular heart rhythm, the third beat may be a premature atrial,nodal or ventricular beat. It can be a regular irregularity or an irregular irregularity. An EKG at the time of the irregularity or capturing it while wearing an outpatient heart monitor would be the ways to determine what your heart rhythm is when you note the extra or third beat.
