I have been having almost unbearable pain in my left foot and somewhat my right. in both feet the pain is in the same location (on the base of my foot under the big toe all the way down to the middle of my arch. i have inserts from a podiatrist..?

Return to your podiatrist to evaluate possible neuromas, and/or tarsal tunnel problems over medial plantar branch of posterior tibial nerve, as a few suggestions. Orthotics are unlikely to need revision, but have the doctor check out.

You may have a condition called hallucinations limits/rigidity. An X-ray is needed to grade the severity of your condition. If that is the cause and the orthotics are not helping, other treatments such as injections and surgery may. Be necessary.
