What is the difference of meaning between mainstay of treatment and first-line treatment?

Mainstay is generally applied to conditions where a single common treatment is the accepted course. Like cleaning and suturing a wound. A first line treatment suggests a condition where the usual 1st choice may need to be avoided, like an antibiotic the patient is allergic to. It also may apply to use of a secondary drug if the first fails to produce acceptable outcome.
