I'm 70 yrs old dm with grade 4 uterine prolapse the whole uterine cervix is protruding outside with symptoms only lower abdominal discomfort and some fecal not urine incontinence, so is surgery amust here or passery ,can i leave it without treatment?

Surgery is not required, but a pessary is difficult to manage. Hysterectomy can be avoided with what is called a retropubic vaginal and uterine suspension. See a reconstructive pelvic surgeon for consultation. Balance the discomfort with the potential cure of same with appropriate surgical correction.
