GERD - GastroEsophageal Reflux Disease (stomach reflux) is the most common cause of epigastric or substernal pain in supine position and it could radiate to the back. Besides GERD, pancreatic disease also causes epigastric pain (stomach pain as you said it) with back radiation. If you feels sour liquid in your throat sometimes when you lay down flat, especially after a meal, you likely have GERD.
