Possibilities include some irritability of superficial nerves in the arm, increased muscle tension, mild inflammation of tendon or muscle. Stress can cause increase in muscle tension. To deal with chronic stress consider stress management therapy.

At your age it is unlikely to be cardiac but your high cholesterol should be a signal for you to educate yourself about the importance of diet, cessation of smoking if this is applicable. Stress management training will be of help for these of your life. I hope you will dedicate yourself to achieving these goals. I wish you good luck.
