A couple random thoughts: 1. Get a gastroenterologist to evaluate problems involving diaphragm and stomach and duodenum. 2. If no answer found, look toward central pathology. Problems with brain involving area postrema can present with hiccoughs, such as neuromyelitis optica (rare and unusual, but worth a look via MRI?) Good luck with this!
