Localized pain, swelling, itching behind the knee is not very suggestive of a blood clot especially since you are on a blood thinner.

If you are on anticoagulants and are taking an adequate dose, blood clot is less likely. However it is important to confirm this with a cardiologist because of the risk of clotting in the valve because of inadequate dosing of the anticoagulants. It may be possible to verify a clot in the leg vein with further testing, but I would depend on your cardiologist to masks that decision.
