It depends. Those with strep throat act as a reservoir for the disease & can pass it to others when they go untreated. The risk of secondary strep complications include rheumatic fever, heart/kidney & nervous system disorders but these occur in only a fraction of infected people. Some are asymptomatic carriers who never have complications. As many as 1/2 sore throats are viral.
