Differential diagnosis should be extended to musculoskeletal causes as well as possibly vascular or lung related issues. Best would be to follow up with his primary care doc so that proper workup can be finalized and conclusions made.

Regular exercise, after clearance by his doctor may help in improving skeletal muscle health and address the musculo-skeletal pain. For healthy diet: http://www.choosemyplate.gov/., exercise 30 minutes/day, drink enough water daily so your urine is mostly colorless, have safe sex, no tobacco alcohol weed or street drugs. Get HPV and Covid vaccines. Wish you good health!
