You have multiple medical conditions which can certainly cause great distress and discomfort but are not life-threatening (although your chest and back symptoms should not be ignored). It is best to discuss this with your doctor.

If you're experiencing true unstable angina I don't think a tranquilizer alone is appropriate treatment. In your case I don't expect that you have atherosclerotic coronary blockages, although it's possible. Have you had a cath? I suspect either vasospastic coronary disorder or microvascular coronary artery disease. These are best diagnosed & treated a cardiology department at a university hospital

If you have unstable angina, it is indeed the most critical issue, and evaluation should not be delayed. The sleep disorder needs to be addressed, and the total body pain may be associated with SLE.
