At age 41, it takes more ovarian stimulation, leading to delays in ovulation and delays in true menstruation. The higher levels of estrogen stimulate additional growth of endometrium, hence the thickness of "22 mm", and spontaneous disintegration of the endometrium, with heavier bleeding/clots as a result. The biopsy is indicated, but uterine cancer is unlikely.
