My dad had rotator cuff surgery 2.5 days ago. a loud noise made him jump today and flinch his arm/shoulder outwards away from his body to the point he yelled out in pain. is it possible he reinjured the repair?

Possible, unlikely : Certainly possible but unlikely. Can’t tell without examining. He needs to be seen by original surgeon

Most likely OK: Following shoulder surgery, there is substantial stiffness, which takes time and therapy to return to normal range of motion. The rehab process can be very painful, and your Dad is at a point of maximal post-op limitations at 2.5 days. Seriously doubt any damage to the surgical site, but check with the surgeon just to be safe.
