Cranial 4th nerve : A bulging area of an artery (aneurysm) around the brain can press on the 4th cranial nerve and cause palsy. While your doctor may feel it is not likely, he/she needs to be sure.

Trochlear nerve: This could involve orbit or retro-orbital location. MRI can reveal structural lesion and help exclude hidden causes which can be treated
