Your pediatrician: Would be your best reference if you need such a note as having knowledge of your previous conditions and likely to be able to accommodate you on a short notice.

2 thoughts: This is an educational site, not a treatment site. -lease see your GP, Urgent Care, or one of our 1-on-1 virtual sites for physical or virtual examination. Second, if you have not been sick it would be unethical to ask someone to be disingenuous.
