If an infant's o2 levels get too low from something smothering - but re obtain oxygen again before going into arrest (needing cpr)- can they have permanent consequences? or would the infant not be able to come back after damaging state starts?

No uniform answer: Infants have a better ability than older kids or adults in tolerating such events. They are also variable in consequence by age of kid/duration of event/other co-morbidities. It is certainly something you want to avoid, but I have seen kids go through such events that seemed very bleek at the time grow into successful adults.
