Possibilities: The head of the bed may have been elevated by a foam wedge placed under the mattress. Feeing at an angle and slower feeds may also help to prevent spit ups. Burp mid and after the feeding. Review growth chart with your pediatrician.

Spitting up: Make sure you are helping her burp every 1/2-1 ounce. Keep her head up while you feed her and make sure she isn't swallowing a lot of air. After feeding, try to keep her up on your shoulder for at least 20 minutes to allow air to rise up while the milk settles. Don't lie her down or put her in an infant seat right away as that may put more pressure on her tummy.
